Titan Submersible: Coast Guard Report Reveals Who Is at Fault for Disaster | E! New

The Titan submersible implosion was preventable according to a new report. Per a press release from the United States Coast Guard, the Marine Board of Investigation or MBI released its report of investigation August 5th regarding the 2023 tourist subtraged that killed all five passengers on board, including Ocean Gates CEO Stockton Rush. In its 300page analysis, Jason Nebauer, the MBI chair, says this marine casualty and the loss of five lives was preventable, adding there is a need for stronger oversight and clear options for operators who are exploring new concepts outside of the existing regulatory framework. I am optimistic the ROI’s findings and recommendations will help improve awareness of the risks and the importance of proper oversight while still providing a pathway for innovation. Per its report, the board determined the primary contributing factors were Oceangate’s inadequate design, certification, maintenance, and inspection process for the Titan submersible. Other factors cited in the report include a toxic workplace culture at Oceangate, an inadequate domestic and international regulatory framework for submersible operations and vessels of novel design, and an ineffective whistleblower process under the Seaman’s Protection Act. The board also found Ocean Gate failed to properly investigate and address known hull anomalies following its 2022 Titanic expedition. The report also includes 17 safety recommendations aimed at strengthening oversight of submersible operations, improving coordination among federal agencies, and closing gaps in international maritime policy. In addition to the CEO, billionaire explorer Haish Harding, French oceanographer Paul Enri Narjoule, businessman Shazada Daywood, and his 19-year-old son Sulamon Daywood also died in the tragedy, which occurred more than 2 miles beneath the surface of the ocean off the coast of Newfoundland during an expedition to visit the wreckage of the Titanic. E-News has reached out to Ocean Gate for comment on the report, but has not yet heard back.

After a two-year investigation into the 2023 Titan implosion, the United States Coast Guard found that OceanGate was at fault for the tragedy that killed five people. OceanGate CEO Stockton Rush, Hamish Harding, Paul-Henri Nargeolet, Shahzada Dawood and his 19-year-old son Suleman Dawood were killed in the incident.
